How to apply for a Companion Card
Follow these steps to apply for a Companion Card:
- Read the eligibility criteria or download the eligibility criteria (PDF) to help you decide if you should apply for a Companion Card.
- If you choose to apply for a Companion Card, you must complete a Companion Card Cardholder Application Form.
- Provide two, high-quality, passport-sized photographs of yourself with your application form. Photographs should not be more than six months old.
- Have both your application form and your photographs signed by the relevant Health Professional / Service Provider as outlined in the application form.

The Cardholder Application Form is also available from Service Tasmania Shops or by telephoning Community, Partnerships and Priorities Division on 1800 009 501.
As part of the assessment process the people involved in your application may be contacted to verify the accuracy of the information provided. This process can sometimes take a number of weeks, particularly if further information is sought from service providers or health professionals.
All information supplied will be handled in accordance with the privacy principles contained in the Personal Information Protection Act 2004 (Tas).
